Health Minister S Subramaniam said the patients are undergoing blood tests to determine if they are carrying the virus.
"So far the confirmed cases of Zika infection in the country still stand at four," he told reporters at a press briefing in his office at the federal government centre of Putrajaya.
Malaysia reported the first confirmed case of Zika on September 1 after a 58-year-old woman who visited neighbouring Singapore was confirmed positive of the virus.
Two days later, on September 3, the health ministry confirmed the first locally transmitted Zika case, after a 61-year-old man in Borneo also tested positive.
The man had not travelled outside the country in the months preceding infection.
The two other victims were a man and his pregnant wife who are living in the southern state of Johor, which borders Singapore where there is an ongoing outbreak of Zika infections.
The World Health Organisation declared a public health emergency earlier this year. It says evidence of Zika transmission has been found in 69 countries and territories.
